WELCOME ABOARD! Now that your application has been accepted, this page will help you with your contract needs.
Please download/read the Outdoor Participants' Manual (pdf format). This will help you understand the policies, rules and regulations. Contracts must be signed and returned with payment by July 1, 2010. We also accept Mastercard and Visa for your convenience. Liability Insurance requirements are available on page 17 of the manual. if you carry your own insurance, have it faxed to (716) 646-5382 for approval. To purchase insurance, fill out the Insurance Application to the right and follow the directions. The forms to the right are for your convenience, they are not necessary for everyone. For example - Only crafters that plan on sampling need the Health Permit.
